Available Form Elements

Form Elements are added to your form to display different content and provide different functionality on your form - these are the available elements.

 

Form Content

This element allows you to add text and images to your form - e.g. for the form heading and instructions - you can add more than one of these elements. This element does not update Donorfy in any way - its function is purely including content on your form to help make it usable. You can add this element multiple times to a form.

Constituent Details

This element allows the entry of title, first name and last name for individual constituents plus optionally ask for an email address, postal address, and phone numbers. You can only add one of these elements to a form.

Communication Consents

This element asks for channel and purpose consents - you can

  • Choose which channels you want to ask permission for
  • Ask people for their preferred channel
  • Ask for permissions for purposes - note that purposes will be included if they are marked to be included on public-facing preference update forms  - see 'Choose which Purposes to Display on the Preference Update Form' in this article Allowing People to Update their Permissions and Preferences

You can only add one of these elements to a form.

Add a Tag

This element allows you to add a tag when a form is submitted - you can

  • Either add the tag automatically or ask a question which if answered yes will add the tag
  • Ask for further information which will be stored in the tag's notes
  • Populate the additional custom fields in the tag, either automatically or according to an answer given.

You can add this element multiple times to a form.

Add an Activity

This element allows you to add an activity when the form is submitted - you can

  • Either add the activity automatically or ask a question which if answered yes will add the activity
  • Ask for further information which will be stored in the activity's notes
  • Add a task which is related to the activity
  • Populate the additional custom fields in the activity, either automatically or according to an answer given.

You can add this element multiple times to a form.

Donation Element 

This allows you to prompt for one-off or recurring donations - this element has 3 tabs which allow you to set up the various options - as follows

  • Display Tab - on this tab you can
    • Choose an amount option as follows
      • Offer a choice of suggested donations - this allows for 3 suggested donation amounts for single, monthly and/or annual gifts, when using this option you can 
        • Optionally enter a description for what the donation amount could be used for 
        • Allow the donor to enter their own ‘other’ amount 
        • For recurring donations, you can choose whether to allow card payments and/or direct debits
        • Optionally you can choose the default selections - i.e. the selections that are made when the form is displayed 
      • Allow people to enter any amount - this will display a single field where people can enter an amount 
      • Use a fixed amount - i.e. this is the amount being requested and the donor cannot change the amount
    • Decide whether to ask the donor to gift aid their donation
    • Display a gift summary message - which will confirm the amount and frequency of the donation
    • Allow a gift message to be entered - any text entered by the donor will be stored in the transaction comments
    • Allow the donor to assign their donation to a selected fund to do this you need to find the fund in Settings | Configuration and enter a suitable text into the ‘Description of fund to show on forms’ 
    • Allow the donation to be connected to an existing constituent - e.g. for tribute or in memoriam donations - these are the options 
      • the existing constituent can either be selected in form element and all donations will be connected to the constituent 
      • Allow the donor to choose a constituent when making their donation with this option 
        • the constituents the donor can choose from are provided by a constituent list - e.g. a list of active tribute fund constituents - 
        • the first 500 constituents provided by the list are selectable by the donor 
        • You can choose the message to display above the list of constituents on the Prompts and Messages tab under the Other section
  • Processing Tab - this tab allows to control how the donation will be processed and how the donor will be thanked
  • Prompts and Messages - this tab allows you to change the standard prompts and messages used on the form 

You can only add one of these elements to a form.

Gift Aid Declaration Opt-In

This element allows a constituent to make a gift aid declaration - for example, you may want to identify people who have donated but not gift aided and send them a link to a form asking them to make a gift aid declaration. 

If you are taking donations from your form you can allow donors to gift aid their donation as part of the donation element.

You can only add one of these elements to a form.

Submit Form

This element validates and submits the form to Donorfy. You can add this element multiple times to a form.
